Care Plan

The Better Access to Psychiatrists, Psychologists and General Practitioners through the Medicare benefit schedule (better access) initiative aims to improve outcomes for people with a clinically diagnosed mental disorder through evidence based treatment. Under this initiative, Medicare rebates are available to patients for selected mental health services provided by GPs, Psychiatrists, Psychologists, eligible Social Workers and Occupational therapists.


Chronic Disease
Management
 

The Chronic Disease Management Care Plan is for patients who have a chronic condition and complex care needs.
Only a GP is able to determine whether a patient's chronic condition would benefit from allied health services and the needs for such services must be identified in the patient's care plan.


Helping Children
with Autism

The Helping children with Autism care plan is for children with Autism, Pervasive Developmental Disorder or disability. This plan consists of 2 parts:

  1. Assessment and diagnosis and development of a treatment plan

  2. Treatment services

 

Summary of Medicare plans


Due to a recent review of Medicare's eligibility and processes specifically related to the Better access to mental health care, helping children with autism and the chronic disease plan (formally known as the enhanced primary care plan) it is important to note that allocation of Medicare plans and criteria for eligibility for plans is the responsibility of your GP or Paediatrician. Therefore all patient Medicare plans used at Chatterbox must be consistent with Medicare's guidelines and criteria.
